7/3/2023 EnchantaVaughn Approved (AM) Upon arrival, client was observed in a pleasant mood while displaying good manners toward his instructors and camp mates. Client is supported for having such a wonderful turn around since last report. Client enjoys being in camp, expressing that he didnât want to go home early because he has been learning and progressing so much in camp. Client was excited to see QMHP, as evidenced by him hugging and expressing joy as he walked to his seat. Client is seen participating in group activity in the library and exhibiting excellent team work. Client has shown he is capable of being a good student if he continues to focus and stay pro active and avoid distractions. (PM) Client transitions into the lunch setting in a good mood, keeping his hands to himself and being respectful to others. Client is seen eating his lunch and supporting his peers. Client understands what is expected of him and shows that by doing what he is suppose to do. Client is learning to communicate his feelings and be open to feedback. Client returned to activity in a happy mood, as evidenced by his smile and upbeat demeanor. (EOD) Client was cooperative during early dismissal, presenting safety and emergency precautions. Client was encouraged to go home and write down 5 things he loves to do in camp. Client displayed feelings of peace and relaxation toward the end of the day, achieving all of his objectives, as witnessed.(AM) QMHP utilized active listening skills as client expressed himself throughout the day. QMHP kept client involved in activities that involved teamwork such as but not limited to team sports in efforts to observe his willingness to utilize the positive social skills he'd been learning. QMHP informed client of his positive behaviors. (PM) QMHP reinforced the importance of positive social communication between client and figures of authority. QMHP modeled positive prosocial behaviors for client in efforts to reinforce his positive interactions with his peers and the mental health professionals. (EOD) QMHP monitored clientâs non-verbal communication with peers in efforts to encourage the use of encouraging communication cues versus discouraging cues. QMHP commended client on having a day with no redirection from anyone. QMHP encouraged client of his abilities to behave himself and be respectful to others. (AM) Client remained soft spoken as he interacted with QMHP and mental health professionals. Client participated in rapport building activity with QMHP. Client stated he was glad to be in camp. Client was able to interact with peers in an age-appropriate manner as evidenced by the client assisting those bullied. Client was observed displaying positive social interactions with his peers and staff. (PM) Client told QMHP that he did not have awareness of his negative impact on his peers and friends through his negative interactions with them. Client expressed that he would walk away when he is angered by a peer instead of lashing out with a negative response. (EOD) Client played appropriately with peers as evidenced by 0 complaints of negative behaviors made by peers or mental health professionals. Client maintained appropriate behaviors. Client displayed body language associated with joy and happiness around his peers. Instructor reported client was respectful and nice and didn't have to redirect client for anything. Client was receptive. Interventions were viewed as successful.
